Licensed Practical Nurse
DEFINITION:This individual provides skilled nursing care to patients in accordance with the physician’s orders under the direction of a registered nurse. Provides intermittent, individualized nursing care to patients in the home seting
LINE OF AUTHORITY:
Reports and works directly under the supervision of the Registered Nurse.
QUALIFICATIONS:
- Currently licensed in the state of Florida.
- Must have physical abilities to adequately perform described duties.
- Ability to learn and apply practical training, follow physician’s orders and produce accurate records of activities and services provided.
- Able to see and hear adequately to respond to auditory and visual requests which relate to the coordination of Agency activities.
- Able to speak in a clear, concise voice in order to communicate direction for Agency activities and staff.
- Demonstrates adequate mental acuity to perform duties stated in job description.
- Demonstrates adequate knowledge regarding exposure and safe handling of body fluids, and chemicals, and some hazardous equipment.
- Demonstrates assessment skills.
- Demonstrates ability to communicate with patients, physicians, and staff.
- Demonstrates motivation to increase knowledge regarding home health care.
RESPONSIBILITIES AND DUTIES:
- Provides patient care assigned by and under the direction of the registered nurse who provides on site supervision as needed, based upon severity of patients, medical condition and training and experience according to the Agency's policies and procedures.
- Prepares clinical and progress notes for the clinical record.
- Assists the MD and RN in performing specialized procedures.
- Prepares equipment and materials for treatment using sterile and aseptic techniques.
- Assists patients in learning self care techniques.
- Report any changes in patient’s condition to the registered nurse with the reports documented in patient files.
- Administers treatments and medications to the patient in the home setting.
- Participates in multidisciplinary approach to patient care.
- Maintains excellent communication with patients, and staff.
- Maintains confidentiality of all patient and Agency matters.
- Performs other duties assigned by the registered nurse pursuant to Chapter 464, F.S.
